Limited quantities of patient brochures are offered for free to WSMA members (100 free brochures every 3 months).

Ordering on behalf of a WSMA member? After creating a new account please contact Justin Dale at jod@wsma.org with the name of the WSMA member affiliated with your organization. We will update your new account once the member physician has been verified (this is a one-time activation).

If you're a patient and would like a single or several copies of a brochure, send us a self-addressed, stamped envelope for each copy requested to: WSMA, 2033 Sixth Avenue, Suite 1100, Seattle, WA 98121, Attn: Patient Materials. Please indicate which item you are requesting.

For all others, you may proceed with ordering at the non-member rate after creating your account.
